The links and files below contain all the paperwork that needs to be completed and turned in to the church office in order for your child to participate in camp. You can get hard copies from the church office, or simply download them from here, print and fill them out and bring them to the church office. Check out the Parent's Page to learn more about CentriKid, and be sure to print a copy of the Parent guide for the week's schedule, information about the theme, and questions to ask your kids after camp!
The cost for camp is $288 per child. The first deposit of $50 per child is due by February 15th to reserve their spot. May 1st is the last day to cancel your spot and recieve your deposit back. The total balance will need to be paid by July 1st. |
Registration FormsThese forms must be printed, signed, notarized, and brought to the church office. You will also need to bring a photocopy of each child's insurance card.
