IMAGE CONSENT FORM. All minors (anyone under 18) must have permission from a parent/guardian to publish images within the church and on church media sites. Physical copies of this form are available at the Children's Check-In desk at the Welcome Center (Portico) entrance.

VISITOR CHECK-IN FORM. When checking in as a visitor, the minimum information required is the guardian's name, relationship, and phone number. The child's name, food, or contact allergy information is also required. This information is required if the caregiver needs to contact the guardian while in the service.
For regular visitors/members, the rest of the information is needed. Physical copies of this form are available at the Children's Check-In desk at the Welcome Center (Portico) entrance. |
